Service Description

Appointments can be scheduled through our website, by phone, or in person. A confirmed appointment constitutes an agreement regarding the scheduled date, time, and type of service.

Cancellation Policy

To provide quality care and minimize wait times, we have established the following policy:

• Cancel or reschedule at least 24 hours in advance to avoid cancellation fees
• Late cancellations (less than 24 hours) may result in a cancellation fee
• Failure to attend without notice results in a no-show fee
• Repeated no-shows may require advance payment for future appointments

Please arrive 15 minutes before your appointment for paperwork. Patients arriving late may be asked to reschedule.

Payment is due at the time of service unless alternative arrangements have been made or insurance billing is authorized. We accept cash, credit cards, and debit cards.

Insurance

We work with many insurance providers and can submit claims on your behalf. However, you are responsible for understanding your coverage, including deductibles and copayments.

DermaPerfect Clinic and staff provide services with reasonable skill and care. However, medical treatment involves inherent risks, and outcomes vary. We do not guarantee specific results.

DermaPerfect Clinic shall not be liable for indirect, incidental, special, consequential, or punitive damages. Our total liability shall not exceed amounts paid for specific services.

We reserve the right to refuse or terminate services under certain circumstances:

• Failure to comply with clinic policies or terms
• Non-payment for services rendered
• Abusive or threatening behavior toward staff or patients
• Providing false or misleading information
• Conditions outside our scope of practice

You may also terminate your use of our services at any time by notifying us.
